Your First Visit
Your initial appointment usually will consist of an examination and review of your treatment options. We will review your personal and medical history and explore your needs and desires. We will want to know how you feel about your teeth, your fears or concerns, and what you want to accomplish in your care. X-rays and other diagnostic records are often needed to plan treatment for your individual needs. Whenever necessary and possible, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan requires planning of appointments to phase the care.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- You will provide us with a current and accurate health history and personal information and consent to examination, necessary tests or records, photographs, medication, local anesthesia, sedation, and recommended treatments as explained by the doctor which includes explanation of the feasible treatment alternatives.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If a prior dentist or physician has taken x-rays, you may request that they forward them to our office. We are able to accept images electronically to help receive the information sooner. We will let you know if more current or additional x-rays are necessary and they can be taken at our facility.
Records
As part of your treatment, we will obtain pre-treatment records to help us understand your pre-existing condition and plan the steps to achieve your goals.
